Program: Age Well Team

Organization: Chatham-Kent Community Health Centres
Description of Services: This program is a free service available to community members who are in their senior years, are at a risk for falls, or are living with a chronic health condition

Team consists of a Registered Nurse, Occupational Therapist, Respiratory Therapist, Kinesiologist and a Physiotherapist

In partnership with the client, the team works to enable clients with the practical tools, knowledge, and skills they need for safe and independent mobility in their homes and community and to optimize their management of chronic health conditions

Services include:
  • home safety assessments
  • individual exercise assessments
  • individual COPD education sessions
  • group exercise programs
  • Master Your Health program
