Description of Services: Provides continuing, medically complex and specialized services to people who have long-term illnesses or disabilities that typically require skilled, technology-based care not available at home or in long-term care facilities * includes private, semi-private and a 4-bed ward rooms

Fees: Daily fee known as "Chronic Care Co-payment" set by the provincial government may be charged while a patient is on one of these units * Fee Reduction: Dependent on financial information provided for co-payment
Eligibility - Population(s) Served: Depending on bed availability, patients can be accepted to a bed at the Chatham Site
Application: Patient must be assessed and accepted by the program director, unit manager, or other specially trained assessors
