Description of Services: Social, recreational, and therapeutic activities are offered for people with Alzheimer's disease and other related dementia, in a safe and stimulating group setting * helps to support family care partners who provide day-to-day care
  • 2 locations: Chatham and Wallaceburg
  • full-time support
  • activities that promote self-esteem
  • nutritious meals and snacks
  • medication support
  • personal care assistance
  • monthly activity calendar
  • respite for caregivers
  • monthly wellness monitored by our Registered Practical Nurse (RPN)
  • foot care provided by our RPN

Fees: Set fee * subsidy available for eligible participants * additional fee for transportation
