| Description of Services: |
10-week group education & exercise program tailored to your individual needs. Each session lasts 2 hours (1 hour education, 1 hour exercise). Work on strengthening your heart and learning ways to self-manage your heart failure, including action planning, stress management, nutrition, self-monitoring, and signs you need to seek medical attention. *This program is also a part of the Sarnia-Lambton Heart Failure Clinic. One-on-one education appointments also available as needed. |
