Program: Pulmonary Rehabilitation

Organization: North Lambton Community Health Centre
Description of Services: Pulmonary Rehabilitation is a disease management, education, and exercise program for people with chronic lung disease. Objectives of the program are to improve symptom management and increase quality of life.

Services provided:
  • medical evaluation, education, and support
  • supervised therapeutic exercise
  • smoking cessation support
  • nutrition and weight management support
  • spirometry (breathing test)
  • 8 week self-management education program
  • weekly exercise groups

Fees: Most medical services covered by OHIP * some fees for other programs
Eligibility - Population(s) Served: Individuals living with chronic lung disease
Application: External referrals accepted. Must be referred by a Primary Care Provider or Specialist to ensure the individual is cleared to exercise. Call for questions or to access the referral.
