Program: Cardiac Rehabilitation Program

Description of Services: Cardiac Rehabilitation is a six-month therapeutic exercise and medical management program for people with known cardiovascular disease. Objectives of the program are to optimize recovery following a cardiac event or procedure, reduce the risk of disease progression, and increase quality of life. Physician or Nurse Practitioner referral is required.

Services include:
  • medical evaluation, guideline optimization, and care coordination
  • 6 month supervised therapeutic exercise
  • self-management education
  • smoking cessation support
  • nutrition and weight management support

The Heart Failure Clinic is an outpatient program that operates on a referral basis, where a nurse practitioner collaborates with physicians and other nurse practitioners to optimize heart failure management. Services are based on individual patient needs, incorporate the latest best practices, and give people the tools to self-manage their disease. Physician or Nurse Practitioner referral is required.
