Program: Let's Go Home with Homeward Bound

Organization: Canadian Red Cross Society - Chatham-Kent Branch
Description of Services: Short term program, temporary bundle of transitional support services intended to assist older adults returning home from a hospitalization including an Emergency room trip.

The following criteria have to be met:
  • over age 55
  • planned or recent discharge from hospital back to home
  • be medically safe for discharge
  • has limited support from family and friends or lives alone
  • willing to accept services

Bundle of services includes:
  • transportation home from hospital
  • up to two additional round trip rides to follow up medical appointments
  • wellness checks
  • socialization visits
  • light housekeeping (sweeping, mopping, dishes, laundry, etc.)
  • grocery shopping
  • meal prep
  • up to four days worth of frozen meals
  • information and referrals to other community supports required to help client's stay at home
