Description of Services: Short-term, person-centered support services available for vulnerable and at-risk seniors, persons with disabilities, and residents of care homes or supported living in Chatham-Kent * all services are client directed, confidential, and delivered at no cost * not a crisis service * voluntary program and service users can withdraw from the program at any time.

•Age-related future planning
•System navigation – community resource and health care
